De heer De Graaf volgde zijn opleiding tot ziekenhuisapotheker in het Zaans Medisch Centrum te Zaandam en het VU medisch centrum te Amsterdam.
Sinds 2006 is hij als staflid verbonden aan de afdeling Klinische Farmacologie en Apotheek van het VU medisch centrum en als ziekenhuisapotheker werkzaam in Ziekenhuis Amstelland.
